When I (the subject) write (verb) this note (object), it is an example of active voice.
Or simply saying that in active voice the subject appears before the object being operated on (verb) by it in the sentence.
When this note (object) is written (verb) by me (the subject), it is an instance of passive voice.
Or simply saying that in passive voice the object appears before the subject who does that (verb) in the sentence.
Notably, the verb usually remains in the middle of the subject & object.
There's however another flexible type of sentence formation in which the verb is also shifted changing its form.
